On May 23rd, the Lawrenceburg Sheriff’s Department was alerted to a case of forgery. The incident began when deputies received a phone call from a member of the Tennessee Department of Correction (TDOC), who reported that an individual had submitted a medical document that appeared to have been tampered with. Following this initial communication, the TDOC representative consulted with a member of the District Attorney’s office and subsequently forwarded an email that contained evidence of the altered medical excuse.
In response to the report, deputies proceeded to Fast Pace Medical in Lawrenceburg to gather more information. There, they met with a Nurse Practitioner who confirmed that he had not authored the additional medical conditions listed on the document submitted by the individual to the Probation and Parole office in Lawrenceburg. Upon reviewing the original medical excuse provided by Fast Pace, it became clear that it did not include any of the extra medical issues claimed by the individual. The discrepancies indicated that the document had indeed been forged after it was obtained from Fast Pace.
As a result of their findings, the Lawrence County Sheriff’s Department took action and issued a warrant for the individual involved in the forgery.
